Taking these medicines together may increase the risk of your blood sugar levels dropping too low (hypoglycaemia). You may also experience more swelling in your ankles or legs.

Cilacar (cilnidipine) is a calcium channel blocker used for blood pressure. These types of drugs can sometimes increase the body's sensitivity to insulin or interfere with how the body manages sugar. Glycomet GP contains two medicines: metformin and glimepiride. Glimepiride is particularly sensitive to other drugs, and adding a blood pressure tablet can occasionally make the glimepiride work more strongly than intended.

You can take these together, but you should monitor your blood sugar levels more frequently, especially when starting or changing doses. Learn to recognise signs of a 'hypo' (low blood sugar), such as feeling shaky, sweaty, dizzy, or hungry. If you notice frequent low blood sugar readings or significant swelling in your ankles, speak to your doctor as they may need to adjust your dosages.

Severe infection, dehydration, or excessive alcohol intake

These conditions significantly increase the risk of lactic acidosis, a rare but life-threatening build-up of acid in the blood. You must stop this medicine and seek urgent medical help if you experience unexplained vomiting, stomach pain, or rapid breathing.

Hypoglycaemia (Low blood sugar)

This medicine can cause your blood sugar to drop dangerously low. You must carry sugar or a glucose snack at all times and be aware that skipping meals or intense exercise increases this risk.

Unstable angina or recent heart attack

This medicine must not be started if you are experiencing unstable chest pain or have had a heart attack within the last month, as it may worsen heart function in the early stages of recovery.

Pomelo y Jugo de Pomelo

Grapefruit juice can significantly increase the levels of Cilacar in your blood, which may lead to a sudden drop in blood pressure, dizziness, or a rapid heartbeat. Avoid consuming grapefruit or its juice while taking this medicine.

Alcohol

Drinking alcohol while taking this medicine can increase the risk of dangerously low blood sugar (hypoglycaemia). It also increases the risk of a rare but serious condition called lactic acidosis. You should avoid or strictly limit alcohol consumption.

Timing with Meals

This medicine should be taken with food (usually with breakfast or your first main meal). Taking it on an empty stomach increases the risk of low blood sugar and can cause stomach upset.
