Can I take Amlodipine and Amitriptyline together?

Taking amlodipine and amitriptyline together can cause your blood pressure to drop lower than intended. This may make you feel dizzy, lightheaded, or faint, especially when standing up quickly from a sitting or lying position. There is also a small risk that both medications could affect your heart rhythm.

Why this happens

Both drugs have the effect of lowering blood pressure (amlodipine is a calcium channel blocker and amitriptyline is a tricyclic antidepressant with alpha-blocking properties). When used together, these effects add up. Additionally, both drugs can potentially interfere with the electrical signals in the heart, though this is less common.

You can take these together, but you should be cautious. Move slowly when changing positions (like getting out of bed) to avoid dizziness. If you feel very dizzy, have a racing heartbeat, or faint, contact your doctor. Your doctor may want to monitor your blood pressure more closely when you first start this combination.

Suicidal thoughts and mood changes

Amitriptyline can increase the risk of suicidal thoughts or self-harm, particularly when first starting treatment or changing doses. Seek immediate medical help if your mood changes or you feel depressed.

Heart conditions (e.g. recent heart attack, arrhythmia)

Do not take amitriptyline if you have recently had a heart attack or have certain heart rhythm problems, as it can affect the electrical signals in your heart.

Severe liver disease

If you have severe liver problems, your dose may need to be much lower and increased very slowly, as amlodipine is processed by the liver and can build up to toxic levels.

Alcohol

Drinking alcohol while taking amitriptyline can make you feel very sleepy and unsteady on your feet. It is best to avoid alcohol, especially when you first start treatment or if your dose is increased.

Grapefruit juice

Grapefruit and grapefruit juice can increase the amount of amlodipine in your body, which may cause your blood pressure to drop too low. This can lead to side effects like headaches, dizziness, or swelling. Avoid consuming large amounts of grapefruit while taking this medicine.

Caffeine (Coffee, Tea, Energy Drinks)

Amitriptyline can sometimes increase the effects of caffeine, making you feel jittery or causing heart palpitations. Conversely, high amounts of caffeine may interfere with the sedative effects of the medicine. Try to keep your caffeine intake moderate.
