A Phase 2, Multicenter, Randomized, Double-blind, Placebo-controlled, Parallel-group Study to Evaluate the Efficacy and Safety of Nipocalimab in Participants with Active Idiopathic Inflammatory Myopathies

This clinical trial is looking at a new potential treatment called nipocalimab for adults living with active idiopathic inflammatory myopathies, commonly known as myositis. These are rare autoimmune conditions where the body's defence system mistakenly attacks its own muscles, causing weakness, tiredness, and swelling. Currently, many people rely on steroids to manage symptoms, but steroids can cause unpleasant long-term side effects. In this study, researchers want to see if nipocalimab can calm the immune system, improve muscle strength, and allow participants to lower their daily steroid dose to 5 mg or less after one year. Half of the participants will receive the study medicine, and half will receive a dummy treatment (placebo) alongside their usual care. The trial is 'double-blind', meaning neither you nor your doctors will know which treatment you are receiving during the study.

¿De qué trata este estudio?

Idiopathic inflammatory myopathies are a group of long-term conditions that cause the body's immune system to attack healthy muscle tissue. People living with these conditions often experience muscle aches, profound weakness, and severe fatigue. Everyday tasks like climbing stairs, lifting objects, or getting out of a chair can become very difficult. Current treatments, such as high-dose steroid tablets, can help control symptoms but often come with difficult side effects when taken over long periods.

This study is testing an experimental medicine called nipocalimab. Nipocalimab works by blocking a specific protein in the blood that keeps harmful antibodies circulating. By clearing these antibodies, researchers hope the drug will reduce muscle inflammation, improve day-to-day physical strength, and allow patients to safely reduce the amount of steroid medicine they need to take.

Because this is a Phase 2 trial, its main purpose is to test how well nipocalimab works and check for any side effects compared with a placebo (a dummy treatment containing simple salt water). By comparing the two groups, doctors can clearly see if any health improvements are genuinely caused by the new medicine.

Taking part helps researchers discover whether nipocalimab could become an effective, muscle-protecting treatment option for people living with myositis in the future.

Puntos clave

  • The trial tests a new drug called nipocalimab for active inflammatory muscle disease (myositis).
  • It aims to reduce muscle weakness and help patients lower their steroid doses over 52 weeks.
  • Participants will be randomly assigned to receive either nipocalimab or a dummy treatment (placebo).
  • The study is for adults aged 18 and over who have active disease symptoms.
  • Joining is entirely voluntary, and you can withdraw at any point without losing your regular medical care.

¿Quién puede ser elegible?

This trial is open to adults aged 18 and over who have been diagnosed with an active inflammatory muscle condition (such as polymyositis or dermatomyositis). 'Active' means you are currently experiencing symptoms like muscle inflammation or weakness that require ongoing medical treatment.

Participants will generally need to be taking steroid medication at the start of the study, as one of the key goals is to see if the new medicine allows your doctor to safely lower your steroid dose.

You may not be eligible if you have certain other serious health conditions, active severe infections, or if you are taking specific medications that could interfere with the study drug. The trial doctor will review your full medical history and perform tests to make sure taking part is safe for you.

¿Podría este estudio ser adecuado para ti?

Responde estas preguntas rápidas para ver si puedes ser elegible. Esto es solo una guía: el equipo de investigación toma la decisión final.

  1. I am aged 18 or older.
  2. I have been diagnosed with an active inflammatory muscle disease (such as myositis).
  3. I currently take steroid tablets (like prednisolone) to manage my muscle symptoms.
  4. I am able and willing to attend regular hospital appointments for infusions and tests over about a year.
  5. I am happy to discuss taking part with my regular doctor or rheumatology specialist.

¿En qué consiste la participación?

Taking part in this study lasts for at least one year (52 weeks). During this time, you will visit the hospital clinic regularly for health checks, blood tests, and assessments of your muscle strength and movement. The study medicine or placebo is given as an intravenous infusion (a slow drip into a vein in your arm). Throughout the study, your medical team will carefully guide you through a planned schedule to gently reduce your daily steroid tablets, especially between weeks 44 and 52. You will also complete short questionnaires about how you are feeling and keep a record of your symptoms. All study-related care, tests, and medicines are provided free of charge.

Riesgos y beneficios potenciales

You may experience improvements in your muscle strength and energy, and you could be able to reduce your regular steroid dose. However, because this is an experimental study, there is no guarantee you will personally benefit, and you might receive the placebo instead of the active medicine. As with any new medicine, there is a risk of side effects, such as mild infusion reactions or a higher chance of common infections. Your health will be monitored very closely throughout the trial, and you are free to leave the study at any time without it affecting your usual NHS care.

Preguntas comunes

What is a placebo and why is it used?

A placebo is an inactive substance that looks just like the real medicine (in this case, simple salty water). It is used to compare results fairly and see if the real drug truly makes a difference.

Will I have to stop taking my normal medications?

No, you will not have to stop everything suddenly. You will continue standard care, and your doctors will only adjust your steroid dose in a planned, safe way while monitoring your health closely.

Can I choose whether I get nipocalimab or the placebo?

No. A computer randomly decides which treatment you receive. Neither you nor your study doctor will know which one you are getting until the trial is finished.

Will taking part cost me anything?

No. The study treatment and all trial-related tests are provided at no cost. Many trials also help cover reasonable travel expenses to and from the clinic.

What happens if my symptoms get worse?

Your safety comes first. If your muscle disease flares up or your health changes, the study doctors can change your treatment or increase your standard medications right away.
