Taking these two medicines together significantly increases the risk of a rare but very serious condition called 'serotonin syndrome'. This happens when there is too much of a chemical called serotonin in your brain. Symptoms can include feeling very agitated, confused, or sweaty, as well as muscle twitching, shivering, a fast heartbeat, or diarrhoea. In severe cases, it can be life-threatening.

Why this happens
Both mirtazapine (Zispin) and venlafaxine (Venladex) work by increasing the levels of serotonin in the brain. When used together, the effect is 'additive', meaning they can push serotonin levels too high for the body to handle safely.
What you should do
Back to contentsYou must speak to your doctor before taking these two medicines together. While doctors sometimes prescribe this combination for difficult-to-treat depression (sometimes called 'California Rocket Fuel'), it must only be done under strict medical supervision. If you are already taking both and feel unwell, confused, or have a fever, seek medical help immediately.
